The next step from my side would be to look into the agent log file and see if there is any additional information.

It seems your w3wp.exe runs under the NETWORK_SERVICE account. I recently had an issue where this built-in user account didnt have privilidges to load our agent dlls - nor did it have the access rights to write agent log files. Can you try - just out of curiosity - to specify a different user account for this application pool and see if that makes a change?

Have you done an iisreset or cycled the application pool and made sure that at least one transaction has been sent to the process?
