After analyzing the flow closely i didnt see an arrow out of the 'Messaging' tier. This means even if each message queue is shown in different tier boxes it may not be like you described in your post.
I think the key is to get a flow out of the messaging tier to your app tier. To do that you need to apply messaging sensors appropriately.
You need to place messaging sensors on agents on either side of your messaging tier. ideally the tagging sensor on the left tier and receiving sensor on the right tier. in your example tagging on BW1 and receiving on BW2. Also check the MQ Tagging option in your profile to see it is set to correctly.

on host health, I am seeing this issue after 6.0( may be wrong).. if transaction flow consists of multiple system profile, then host health of All Hosts of only current profile( selected in filter) is shown. others will be show in grey.. I
On DB, I think all DBs are group by type of of DB like Oracle, MSSQL, MYSQL etc..

Same is true of DB as well
For DB there will one box for each type of DB, not for every DB.This could be an RFE, you may want to request Andi to move this to RFE section.
The flow looks good, you may have an issue with the permission of the user running the .Net process. The agent is not able to capture the host and process details.

You mean to say have one box for each message queue? That will be nice.
could you please post (after hiding any text) the current transaction flow view to see how it shows
