I logged in and found it strange that there are no recent logs for this monitor. If you have access to the collector server, I would recommend you login and check that the file system is not full, and see if maybe there are logs present on the server that are not showing up under the "System Information" dashlet. It would be good to get more details on why this monitor is failing for 4 out of 7 measures currently (although I also noticed that the 4 measures that are currently failing, apparently have never returned any measures).
