I see this issue very often during the day but it isn't related to the system because it is a pre-production environment with a very low load.

I don't see the 34.32 second figure you're referring to in any of the dashboards I looked at, including the Transaction Flow. Could you post a screenshot?
As for IIS, the delay could be in an IIS module, or the request could be getting stuck in an IIS or ASP .NET queue, or perhaps IIS is short of threads. As you say, it's not likely to have anything to do with data transmission, since the data size is so small. Do you see this problem all the time, or only under load? Are you running a lot of app pools / worker processes on the system?
There is an IIS monitoring plugin here: IIS Monitoring Plugin which may help. You do need to make sure that the Dynatrace Collector has the right permissions to access perfmon on the system(s) you're monitoring, as described here: How To Retrieve Performance Metrics with Windows Performance Monitor

Hi,
It's since few weeks we have UEM component installed but in many case I have some difficult to well diagnostics the user action purepaths. I attach one of these cases which the user action:
1) purepath has taken 32929 ms and quite 97% are spent in IIS. It's not clear to me for what to do because they have transferred only 23 Kbytes
2) Again the transaction flow says 34.32 sec on the user, so a difference of 2 sec respect the user action purepaths.
I attach the relative purepath e and we are using 6.1 version.
